CHIEF COMPLAINT: Severe oral pain

BACKGROUND: In her 70’s and bedridden, the client was not able to visit a dentist for several years. When she developed an abscess and chipped a front tooth she grew desperate to arrange care. In New York City, where office space is at a premium, there is little accommodation for in-coming ambulances in mixed-use buildings and liability issues forbid non-emergency use of service elevators for stretcher-bound patients. Most in-hospital dental departments are not prepared to perform procedures normally done on an outpatient basis and few mechanism exist for the hospital to be reimbursed for these services. Despite referrals from former dentists, no one was able to find a dentist’s office that could accommodate this patient. One concerned office manager Googled House Call Dentists and within hours of contact we were able to see this patient in her home.

We arrived at her bedside with a mobile dental setup that included everything from a compressor and x-ray machine to rinse cups. After examination, we carefully explained the proposed treatment plan, a tentative schedule, and estimated cost.

Because the patient was able to cooperate in the procedures, all of the work was done with local anesthesia over three visits, entirely at her bedside.
